Start-Up / On-site Service
Review the Start-Up / Service Guidelines attached to the Start-Up/Service Scheduling and Authorization Form. Start-up is to be scheduled a minimum of two (2) weeks in advance by signing and faxing the authorization form to Drivecon, Inc. (847.855.9650), along with a Purchase Order referencing the project and/or customer purchase order number. Drivecon, Inc. will confirm this request by countersigning and returning the form.
